Job Summary
Conduct client intakes, assessments, and individualized goal planning to guide individuals and families experiencing homelessness toward securing and maintaining safe, affordable housing. Provide weekly case management sessions, ongoing advocacy, and crisis intervention while collaborating with property management to ensure program compliance. Assist clients in understanding leases, meeting tenant responsibilities, and navigating external appointments through referrals and support. Maintain accurate client records, data entry, and required monthly/quarterly reporting. Build relationships with community partners and local landlords to expand housing access. Perform exit interviews and complete required documentation when clients transition from the program. Participate in meetings, trainings, and the program's on-call rotation.
Required Qualifications
- Bachelor's degree in human services or related field with two years' experience
- Associate degree equivalent with four years' experience
- In-depth knowledge and understanding of the social welfare system
- Valid driver's license
- Clean driving history
- Reliable personal vehicle
- Proof of insurability
- Current FBI clearance
- Current criminal clearance
- Current child abuse clearance
- Ability and willingness to obtain FBI, criminal, and child abuse clearance
