Housing Stability Case Manager - Homelessness Prevention

Job Summary

Develop case management plans for adults and children, updating them monthly with client signatures to address housing stability, income growth, and family support. Meet weekly with families on caseload to review progress, coordinate employment supports, and network with community resources for housing, substance abuse treatment, and mental health referrals. Complete required assessments, maintain regular communication per contract guidelines, and obtain signed release of information forms to facilitate referrals to treatment and vocational programs. This role supports the Homelessness Prevention Program at Community of Hope's Conway Health and Resource Center in Southwest, DC.

Required Qualifications

  • Bachelor's degree in social work or related field
  • 4 years social service experience
  • Minimum one year work experience in social service or related field
  • Strong interpersonal, conflict-resolution, and organizational skills
  • Ability to work independently and as a team
  • A valid driver's license
  • Regular access to a car
  • Proof of auto insurance

Desired Qualifications

  • Prior experience working with individuals or families experiencing homelessness and/or living in poverty
