Job Summary
Provide culturally responsive, trauma-informed case management services and referrals to community resources to remove barriers to maintaining or stabilizing housing. Conduct in-person home visits, complete daily documentation, interpret reports and assessments from providers, and maintain complete and accurate client records. This in-office position requires an AA degree in Social Work, Psychology, Sociology, Human Services, or related field with at least two years of relevant work experience, along with a valid Minnesota driver's license and proof of insurance.
Required Qualifications
- AA degree in Social Work, Psychology, Sociology, Human Services, or related field
- At least two years of relevant work experience
- Valid Minnesota driver's license
- Proof of insurance
- Ability to conduct in-person home visits
Desired Qualifications
- BA degree in a related Human Services field
- At least one year of relevant work experience (with BA)
- Strongly relevant work experience of at least four years (substituting for BA)
- Solid understanding of homelessness
- Solid understanding of mental health issues
- Solid understanding of substance abuse
- Solid understanding of navigation of housing resources (including eviction prevention)
- Solid understanding of domestic violence
- Solid understanding of community resources
