Job Summary
Provide culturally responsive, trauma-informed case management services and referrals to community resources to remove barriers to maintaining or stabilizing housing. Conduct daily documentation, interpret reports and assessments from providers, and maintain complete and accurate client records. Perform in-person home visits as required. This in-office position offers potential to transition to hybrid after demonstrating competency and completing training. Applicants must possess a valid Minnesota driver's license and proof of insurance. The role supports the Amherst H. Wilder Foundation's mission to create lasting, positive change through direct services, research, and advocacy.
Required Qualifications
- AA degree in Social Work, Psychology, Sociology, Human Services, or related field
- At least two years of relevant work experience
- Valid Minnesota driver's license
- Proof of insurance
- Ability to conduct in-person home visits
Desired Qualifications
- BA degree in a related Human Services field
- At least one year of relevant work experience (with BA)
- Strongly relevant work experience of at least four years (substituting for BA)
- Solid understanding of homelessness
- Solid understanding of mental health issues
- Solid understanding of substance abuse
- Solid understanding of navigation of housing resources (including eviction prevention)
- Solid understanding of domestic violence
- Solid understanding of community resources
