Job Summary
Outreach and engage individuals referred to the program who are currently without stable housing. Complete screening and intake processes, assess housing stabilization needs, and develop participant-led housing service plans. Assist participants through the housing process, including completing application and lease-up paperwork, accessing funding for move-in costs, and obtaining furnishings. Complete necessary documentation, maintain participant files, and actively participate in multi-disciplinary team meetings. Develop positive relationships with collaborative partners, property managers, and external providers to support successful housing stabilization and prevent eviction. Provide ongoing home and community-based services using strengths-based case management and assertive engagement techniques.
Required Qualifications
- Degree in social-service related field
- QMHA credential
- CADC credential
- CRM credential
- PSS credential
- Professional experience working with Elders experiencing homelessness
- Knowledge of housing resources and programs
- Knowledge and understanding of recovery-based, holistic approaches to addressing behavioral health needs
- Knowledge and skill in working in a multi-cultural and multi-disciplinary environment
- Ability to engage and interact positively with individuals and families to promote strengths and improve health and well being
- Access to reliable transportation
- Current driver's license with a clean driving record
- Proof of auto insurance
- Ability to pass a pre-employment or for cause drug tests
- Ability to pass criminal background and DMV checks
- Must be able to obtain QMHA/CADC/CRM or PSS credential within 6 months of hire
