Job Summary
Provide case management services to a caseload of up to 15 clients and deliver housing assistance within the facility. Manage and develop facility systems, conduct periodic groups, and complete all required charting paperwork and housing assessments per funding sources. Perform Housing Assistance Referrals and complete HRA 2010-E's (CAPS). Attend management, team, and staff meetings while providing facility assistance as needed. Requires CASAC-T certification, minimum two years' experience in a residential setting, and proficiency in Microsoft Office.
Required Qualifications
- GED/High School Diploma or Associates Degree
- Minimum 2 years' experience with population in a residential setting or related field
- Proficient in Microsoft Office and related programs
- CASAC-T
Desired Qualifications
- Either CASAC or Bachelor degree
