Job Summary
Housing Specialist will review residents’ needs to link community programs and services, serve as a multidisciplinary assessment team leader to coordinate care, facilitate care conferences with residents and families, and plan/coordinate social, educational, and recreational programs. The role involves preparing and distributing newsletters to inform residents about planned activities and requires strong written/verbal communication, adaptability to evolving program needs, and proficiency with Microsoft Office. Bilingual ability is a plus, with an emphasis on supporting older adults and individuals with disabilities in the South Shore Plymouth locations.
Required Qualifications
- At least two years of experience in the human services field, preferably with the elder population
- Excellent written and verbal skills
- Excellent computer skills
- Proficient with Microsoft Office Suite or related software
