Job Summary

Conduct mock interviews, facilitate housing readiness groups, and escort clients to appointments to prepare residents for permanent housing. Secure necessary documents, review HRA 2010e applications, and submit housing applications in coordination with case management. Identify and match clients with appropriate housing options, including fair market landlords, while tracking progress toward housing readiness. Maintain relationships with housing providers and communicate updates via progress notes and staff meetings. This role supports Breaking Ground's mission to help formerly homeless individuals achieve long-term housing stability across New York City and upstate New York.

Required Qualifications

  • Bachelor's degree or equivalent experience
  • Two years' experience
  • Must be able to use public transportation to accompany clients to appointments
  • Must be willing and able to engage in moderate physical activity to engage with street homeless individuals
  • Such activities include and are not limited to moderate walking and climbing

Desired Qualifications

  • Substance abuse, homelessness, motivational interviewing, mental health conditions
  • knowledge of supportive housing and DHS housing programs
  • Previous experience working with homeless/formerly homeless populations, adults with substance abuse /dual diagnosis
  • Excellent interpersonal and problem-solving skills
  • Strong writing and verbal communication skills
  • Bilingual in Spanish and English
  • Proficiency with Microsoft Office
  • Experience with HRA 2010e, AWARDS /Foothold and CARES
