Housing Specialist
Job Summary
Housing Specialist on the Intensive Care Case Management (ICCM) team in Philadelphia, PA. Responsible for helping participants access housing resources, conduct apartment hunting, negotiate with landlords, develop individualized recovery plans, coordinate referrals, and advocate for housing stability while supporting independent living and daily living activities. Responsibilities include sourcing market-rate/affordable housing, creating a resource directory, facilitating groups on preparedness for independent living, coordinating with landlords, agencies, and healthcare providers, and documenting services in the electronic health record. Requires collaboration with a multidisciplinary team, trauma-informed and recovery-oriented practices, and willingness to engage in on-call coverage and local travel.
Required Qualifications
- Bachelor’s degree in Human Services/Social Services, Healthcare, or a related field and 3 years of experience in the Mental Health/Substance Abuse field.
- Proficient communication and analytical skills; must be detail oriented.
- Knowledge of mental health systems and supports with some knowledge of multiple support systems for substance abusers.
- Ability to work independently and as part of a focused team.
- Possession of a valid driver's license with an acceptable driving record
