Job Summary
Housing Specialist role providing ACT-based housing and treatment services; conducts community outreach, psychoeducation, and housing coordination; responsibilities include community treatment contacts, daily team meetings, documentation per NYS OMH and NYC DOHMH licensing and Medicaid guidelines, collaboration with client support networks, housing applications (HRA2010e), preparing clients for housing interviews, escorting and supporting in housing interviews, and on-call crisis intervention. Requires a Bachelor’s degree and experience working with individuals with serious mental illness and/or co-occurring substance use disorders; preferred fluency in Spanish.
Required Qualifications
- Bachelor’s degree
- Experience working with individuals with serious mental illness, and/or co-occurring substance use disorders
- Experience with HRA2010e supportive housing application and familiarity with housing resources for ACT-eligible population
- A team player, highly organized and self-motivated
- Proficient in computers
- Individuals with lived experience are encouraged to apply.
Desired Qualifications
- Experience working with individuals involved in the criminal legal system
- Fluency in Spanish
