Job Summary
The Housing Specialist/Case Manager II will deliver essential services for clients accessing the HOPWA Rent Subsidy Program, focusing on housing stabilization, benefits advocacy, and health support. They will assess participant needs and develop personalized service plans while coordinating with multiple agencies to ensure compliance with program requirements.
Required Qualifications
- Bachelor’s degree in Social Work or related field
- Minimum of two years related experience in case management
- Sensitivity to HIV/AIDS services and homelessness issues
Desired Qualifications
- Experience in crisis intervention
- Bilingual in English/Spanish
- Knowledge of substance use disorders
- Strong communication skills
