Job Summary
Housing Specialist to support older adults and individuals with disabilities in the South Shore area. Lead multidisciplinary assessment teams, coordinate services, facilitate care conferences with residents and families, plan and coordinate social, educational, and recreational programs, and prepare newsletters to inform residents. Requires at least two years in human services (elder population preferred) and a Bachelor's degree in social work or related field preferred. Strong written and verbal communication, proficiency with Microsoft Office; bilingual skills are a plus. 35-hour work week, typical M-F, with great benefits and opportunities for professional development.
