Job Summary
Assess participant needs for housing services and develop opportunities for consumers with co-occurring mental illness and substance abuse disorders. Conduct comprehensive life assessments, organize structured group and individual activities, and provide direct service interventions including supportive counseling and symptom management. Coordinate Individual Treatment Planning processes, monitor progress, and obtain documentation for housing inspections and clinical records. Establish relationships with community providers, procure housing resources, and provide transportation to meetings. Perform on-call crisis intervention covering nighttime and weekend hours while maintaining 80% field footwork. Maintain detailed clinical records and filing systems.
Required Qualifications
- Bachelor's Degree in mental health field
- three years equivalent work experience in mental health and / or substance abuse services
- Two to three months OJT
- valid driver's license
- clean driving record
- ability to work independently with minimum supervision
- field foot work 80% of the time
