Job Summary
The Housing Services Case Manager will provide comprehensive support to Transition Age Youth (TAY) aged 18 to 24 who are homeless or at risk. Responsibilities include case management to assess and coordinate services, supervise and ensure the safety of residents, assist participants in accessing necessary resources such as benefits, treatment, and educational services, and maintain communications with various stakeholders. The role requires strong organizational skills, flexibility, and the ability to work both independently and in teams.
Required Qualifications
- Minimum of 2 years’ experience working with vulnerable populations and at-risk youth in a public or private child welfare social services setting
- Current valid California driver’s license and current automobile insurance
- Maintaining a clean driving record acceptable to the organization’s insurance company
- Immediate access to a vehicle during work hours
