Job Summary
The Case Manager for Penny Lane’s Permanent Supportive Housing Program provides direct case management and support to 20 residents formerly experiencing chronic homelessness. Responsibilities include assisting with obtaining services such as benefits, housing linkages, and substance use disorder treatment. The role requires developing housing stabilization plans, conducting apartment checks, and ensuring residents' safety and self-sufficiency. Candidates should have at least 2 years of experience working with vulnerable populations, particularly at-risk youth, and possess a valid California driver's license.
Required Qualifications
- Minimum 2 years’ experience in social services or related field
- Valid California driver’s license and clean driving record
- Full COVID-19 vaccination and boosters
