Job Summary
Conduct weekly home visits, escort tenants to appointments, and create mutually agreed-upon service plans based on initial and ongoing assessments. Manage a caseload of up to 20 households by providing assistance with benefits recertification, subsidy maintenance, and financial literacy workshops. Connect tenants to rental assistance, primary care, behavioral health services, and job training resources while collaborating with building management to address lease responsibilities and safety concerns. Provide DV/IPV crisis intervention and support for school attendance. Maintain record keeping in compliance with City and State agency standards.
Required Qualifications
- Bachelor's degree in Social Work
- Bachelor's degree in related field
- MSW preferred
- Minimum 3 years of experience working with formerly homeless individuals and families
- Housing-based case management experience
- Demonstrated understanding of behavioral health
- Familiarity with best practices in mental health and substance use recovery
- Best practices in mental health and substance use recovery rooted in harm reduction
- Ability to positively engage with residents who present with special needs
- Strong collaborative skills
- Ability to work successfully with a wide range of internal and external stakeholders
- Excellent interpersonal skills
- Creativity
- Problem-solving ability
- Proficiency with Microsoft Office Suite programs including Word, Excel, Outlook, and Microsoft Teams
- Experience with electronic case records
- Ability to learn new programs
