Job Summary
Conduct assessments, planning, and home visits to address housing needs and develop service plans for clients. Assist individuals in locating affordable housing, completing applications, and accessing medical and psycho-social services. Serve as client advocates with landlords and housing programs while ensuring compliance with federal and state regulations. Maintain detailed progress notes, manage case logs, and participate in case conferences and program meetings. Provide crisis intervention and support clients with independent living skills such as budgeting and banking.
Required Qualifications
- Bachelor's degree in health, Education, or Human Services
- At least two years of experience with HIV/AIDS, chemical dependency, or mental illness
- Associates degree in Health or Human Services
- At least 3 years' experience with HIV/AIDS, chemical dependency, or mental illness
- Bilingual (English/Spanish)
- Valid NYS Driver's license
- Registered and reliable vehicle
- Ability to prioritize assignments, plan, and complete work projects with minimal direction
- An ability to work efficiently and effectively and meet deadlines
- An ability to work under pressure
- Excellent verbal/written skills
- Ability to maintain confidentiality
- Ability to demonstrate behaviors and attitudes which support agency mission, philosophy, and policies
- Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
