Job Summary
Conduct thorough screenings and assessments with members and families to gather functional, environmental, financial, and health information. Develop rapport to understand housing needs and create feasible Individualized Housing Support Plans based on Community Support Assessments. Offer personalized case management support, including life skills training, job preparation, and financial literacy guidance to promote self-sufficiency. Act as a liaison between landlords and tenants to resolve issues, conduct post-housing home visits, and coordinate case conferences. Maintain member records, track outcomes in databases, and submit regular progress reports to the Supervisor. Perform additional duties as assigned while utilizing resources effectively under emotional stress.
Required Qualifications
- High school, diploma or GED
- Minimum of 1 year experience in case management, patient care, customer service, call center, or patient care
- Valid California driver's license
- Valid vehicle insurance
- Excellent communication, interpersonal, customer service, active listening and organizational skills
- Computer skills for documentation, email and chat support
- Proficient speaking, reading, and typing abilities and possess strong proofreading skills
- Proficiency in working independently and collaboratively in a team to provide member care
- Proficiency in multitasking, organization, financial management, planning and attention to detail
- Capability to perform under circumstances of possible emotional stress and conflict, including dealing with uncooperative members and families
- Ability to utilize resources effectively for problem-solving while maintaining composure under pressure in a compassionate manner
- Must have an empathetic and warm-hearted nature
- Must be able to travel using personal vehicle to complete outreach visits
- Must successfully complete and maintain current Tuberculosis (TB) testing and clearance in accordance with company policy and applicable local/state health regulations as a condition of employment
- Able to lift up to 15lbs
- Must be able to lift 50 lbs
