Job Summary
Conduct thorough screenings and assessments with members to gather functional, environmental, and housing information, then develop sustainable Individualized Housing Support Plans. Cultivate relationships with community programs and leaders to secure permanent housing while offering crisis intervention for the homeless. Provide personalized case management, life skills training, and financial literacy support to guide participants toward self-sufficiency. Act as a liaison between landlords and tenants to resolve issues, conduct post-housing home visits, and coordinate case conferences with interdisciplinary teams. Maintain member records, track outcomes in databases, and submit weekly and monthly progress reports to the Supervisor.
Required Qualifications
- High school, diploma or GED
- Minimum of 1 year experience in case management, patient care, customer service, call center, or patient care
- Valid California driver's license
- Valid vehicle insurance
- Excellent communication, interpersonal, customer service, active listening and organizational skills
- Computer skills for documentation, email and chat support
- Proficient speaking, reading, and typing abilities and possess strong proofreading skills
- Proficiency in working independently and collaboratively in a team to provide member care
- Proficiency in multitasking, organization, financial management, planning and attention to detail
- Capability to perform under circumstances of possible emotional stress and conflict, including dealing with uncooperative members and families
- Ability to utilize resources effectively for problem-solving while maintaining composure under pressure in a compassionate manner
- Must have an empathetic and warm-hearted nature
- Must be able to travel using personal vehicle to complete outreach visits
- Must successfully complete and maintain current Tuberculosis (TB) testing and clearance in accordance with company policy and applicable local/state health regulations as a condition of employment
- Able to lift up to 15lbs
- Employment is contingent upon a successful pre-employment drug screening and background check
