Job Summary
Identify new and existing patients to determine program eligibility by reviewing registration information, social determinant screenings, and income. Maintain a caseload of eligible participants, working with them to eliminate barriers to housing stability through meetings in the clinic, community, or at patients' homes. Complete a housing stability plan with each participant and assist them through every step of the housing process, including searches, applications, move-ins, and ongoing support. Act as an advocate with valid Release of Information authorization, facilitate warm handoffs to Outreach, clinical divisions, and community specialty services, and maintain collaborative linkages with local social services agencies and housing authorities. Complete encounter documentation for all work done in a timely manner.
Required Qualifications
- Associate degree in a health or social services related field or 2 years equivalent experience
- Clearance through the Idaho Department of Health and Welfare Criminal History background check
- Demonstrated willingness and desire to work with low-income and unhoused populations
- Demonstrate an understanding of the barriers for people with behavioral health and substance misuse problems
- A working knowledge of Boise area social service and housing resources
- Ability to work as a team member
- Good verbal and written communication skills
- Reliable and courteous
