Job Summary
Conduct comprehensive needs assessments within seven days of shelter entry, develop individualized service plans, and manage a caseload of 22–30 individuals to achieve housing stability. Provide crisis intervention, facilitate warm handoffs to housing providers, and coordinate referrals for employment, benefits, and healthcare services using trauma-informed care and harm reduction strategies. Maintain accurate client data in HMIS, complete navigation assessments, and collaborate with external agencies including landlords and social workers. Assist clients in obtaining necessary documents and developing coping skills while working independently in a fast-paced team environment.
Required Qualifications
- Bachelor's Degree in Human Services or related field
- At least 1 year of experience working with homeless individuals and mental health
- Individuals with less than a BA/BS degree but with at least 2 years of direct life experience working with long-term homeless, low-income, and diverse populations and have a working knowledge of mental health and recovery
- Possess a high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs
- Bilingual in Spanish
- Ability to work effectively with a diverse population
- Ability to plan, organize and prioritize duties
- Ability to perform crisis intervention as needed
- Ability to clearly communicate information and instructions verbally and in written form
- Ability to maintain a positive, professional and safe environment while on duty
- Ability to establish and maintain effective working relationships with others
- Ability to lift up to 50 pounds
- Reliable transportation
