Job Summary

Conduct weekly case management for 20-30 clients, including intakes within 24 hours, comprehensive assessments, and crisis intervention. Assist clients with securing housing by obtaining vital documents, submitting HMIS referrals, and providing warm hand-offs to permanent housing providers. Maintain up-to-date case notes, physical files, and accurate data entry while collaborating with community partners, landlords, and social workers. Deliver trauma-informed care, harm reduction strategies, and life skills coaching to support housing stability for seniors.

Required Qualifications

  • At least 2 years of Case Management experience working with homeless individuals or Senior Citizens
  • Bachelor's Degree in Human Services or related field
  • Individuals with less than a BA/BS degree but with at least 4 years of experience working with long-term homelessness, low income, senior citizens or diverse populations
  • Working knowledge of mental health, behavioral health and recovery services
  • A valid AZ driver's license
  • Proof of insurance
  • A reliable vehicle
  • Excellent communication and interpersonal skills
  • Fluency in Spanish
  • Strong organization and time-management skills
  • Ability to maintain a positive and professional environment
  • Proficiency in Microsoft Office programs
  • Ability to communicate effectively both verbally and in written form
  • Detail-oriented with the ability to manage multiple tasks and meet deadlines
  • Must have excellent problem-solving skills
  • Ability to perform crisis intervention with a trauma-informed modality
  • Current CPR certification
  • Ability to lift, bend, push, and pull up to 50 pounds
  • Ability to work in a fast-paced environment
  • Exemplify flexibility, adaptability to sudden changes, trustworthiness, reliability, and empathy
  • Ability to model professionalism and respect at all times
  • Willingness to gain continual knowledge and understanding of Senior Housing, Community Resources, Adult Rehabilitative Services, Mental & Behavioral Health Services and Substance Abuse Treatments

Desired Qualifications

  • Fluency in Spanish is of significant value
