Job Summary
Conduct street outreach and navigation to identify individuals and families at risk of homelessness within Fresno and Madera County. Perform intake and assessment to determine eligibility, then manage caseloads by developing service plans and tracking data in the Clinical Data System. Link clients to housing resources, apply for vacancies, and facilitate warm handoffs to secure permanent housing. Transport clients in a company vehicle and maintain relationships with property managers. Provide weekly updates to the Master by Name List and participate in case conferencing. Deliver all services in a linguistically and culturally appropriate manner using interpreters as needed.
Required Qualifications
- Have a strong working knowledge of housing services, supportive services, and community resources.
- Excellent communication skills.
- Ability to exercise good judgment and discretion.
- Ability to work well in a team environment.
- Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality.
- Professional appearance and demeanor.
- Must be sensitive to populations served.
- Valid driver's license and ability to obtain and maintain WestCare's vehicle insurance is required.
- CPR Certification, First Aid Certification, and an annual Tuberculosis test.
- Two (2) years of case management/direct outreach experience is required.
- Graduation with a two-year degree in Addiction Counseling, Psychology, Social Work, Community and Human Services, Rehabilitation Counseling, or related field is required.
- Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation
- Some travel is required.
Desired Qualifications
