Job Summary
Provide key-worker support to adults with mental health needs or housing challenges to build independence and maintain accommodation. Complete needs assessments, create person-centred support and safety plans, and review progress regularly to help individuals work toward their goals. Liaise with mental health teams, community groups, and partner agencies to ensure individuals receive the right external support for accessing benefits, managing daily living tasks, and engaging with local services. Maintain accurate records, uphold safeguarding procedures, and contribute to service improvements while representing the service at community events.
Required Qualifications
- Access to a vehicle
- Full UK driving licence
- Valid business-use motor insurance
- Housing Support Worker/Support Worker experience
- Experience of providing support to people with a mental illness
- Experience of providing care and/or support services within another social care setting in either a paid or voluntary capacity
- Excellent administration, organisation and planning skills
- Competent in the use of IT systems including Microsoft applications, email and internet
