Job Summary
Engage individuals with disabilities and those in recovery to assist them in meeting basic needs, including finding safe and affordable housing. Resolve housing crisis situations by serving as the internal expert on local resources and collaborating with Continuums of Care to establish networks for stable housing. Reach out to unengaged households experiencing homelessness, refer clients to behavioral health and employment services, and build relationships with landlords. Utilize SOAR training to help individuals apply for benefits and identify readiness for permanent housing options. Complete timely data collection, participate in Coordinated Entry meetings, and reengage disconnected participants to prevent returning to homelessness.
Required Qualifications
- Bachelor's in Social Work, Psychology, Sociology, or other relevant case manager experience in housing
- Associate of Applied Science in Behavioral Health Support from an approved institution eligible to provide community support services
- Qualified to provide Community Support through specified educational background and experience as defined in community-support qualifications
- Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check
- Current driver's license, acceptable driving record, and current auto insurance
- Completion of the SOAR programs
- Completion of Housing Liaison training materials provided by the Department of Mental Health
- Completion of the HUD Quality Standards quiz
