Job Summary
Provide Housing Transition Navigation Services and Housing Tenancy and Sustaining Services to high-risk patients experiencing homelessness, chronic disease, and serious mental illness. Collaborate with an interdisciplinary team including primary care providers, RNs, social workers, and health coaches to support members enrolled in Community Support or In Lieu of Services programs. Navigate medical and social barriers while advocating for non-Gardner and Gardner Health Services clients within the Downtown Resource Center. Requires bilingual English/Spanish proficiency, knowledge of CPS/APS reporting, and experience in discharge planning. Full-time role with competitive hourly pay.
Required Qualifications
- Bachelor's degree in Social Work or Psychology
- Excellent written and verbal communication skills
Desired Qualifications
- Two (2) years' of experience in the provision of social services and/or discharge planning in an acute care setting
- Bilingual English/Spanish
- Ability to work with a diverse ethnic and multi-cultural population
- Knowledge of CPS and APS mandatory reporting laws
- Knowledge of medical terminology, Medicare, Medi-Cal, and county and community agency regulations
- Advanced communication skills in advocacy, negotiation and conflict resolution
- Computer skills including proficiency in medical practice management systems, internet use, and Microsoft office applications
- Knowledge and understanding of human behavior, development of personality, and adaptation to illness
- Knowledge effects of hospitalization on patients and families
- Ability to set and meet deadlines
- Creative and flexible team player
- Must be comfortable in a fast-paced environment
- Demonstrated working knowledge of OSHA and HIPAA guidelines
