Job Summary
Conduct intensive case management for older adults (50+) with HIV, managing a caseload of up to 50 clients through housing navigation, assessments, and crisis intervention. Develop and monitor Individual Service Plans to secure permanent support housing, short-term rentals, and utility assistance via the Coordinated Entry System. Coordinate with medical providers and external services to facilitate Medicaid, SNAP, and other benefits while arranging transportation and follow-up appointments. Maintain comprehensive documentation in electronic health records and collaborate on program evaluation and tool development. Requires willingness to travel throughout the Chicago metropolitan area.

- Strong leadership, communication, organizational and interpersonal skills with a diverse range of individuals, organizations, and communities
- Willingness to travel throughout the Chicago metropolitan area
Desired Qualifications
- Bachelor's degree in social work, counseling or related field
- Knowledge of older adult services
- Knowledge of local housing resources and program requirements
- Knowledge of tenancy rights and landlord-tenant laws
- Familiarity with harm reduction, LGBTQ affirming, and trauma-informed philosophies of care
- Bilingual Spanish-speaking
- Bi-cultural
- Work experience with immigrant, asylum-seeking, and refugees
