Job Summary

Conduct intensive case management for older adults (50+) with HIV, managing a caseload of up to 50 clients through housing navigation, assessments, and crisis intervention. Develop and monitor Individual Service Plans to secure permanent support housing, short-term rentals, and utility assistance via the Coordinated Entry System. Coordinate with medical providers and external services to facilitate Medicaid, SNAP, and other benefits while arranging transportation and follow-up appointments. Maintain comprehensive documentation in electronic health records and collaborate on program evaluation and tool development. Requires willingness to travel throughout the Chicago metropolitan area.

Required Qualifications

  • Strong leadership, communication, organizational and interpersonal skills with a diverse range of individuals, organizations, and communities
  • Willingness to travel throughout the Chicago metropolitan area

Desired Qualifications

  • Bachelor's degree in social work, counseling or related field
  • Knowledge of older adult services
  • Knowledge of local housing resources and program requirements
  • Knowledge of tenancy rights and landlord-tenant laws
  • Familiarity with harm reduction, LGBTQ affirming, and trauma-informed philosophies of care
  • Bilingual Spanish-speaking
  • Bi-cultural
  • Work experience with immigrant, asylum-seeking, and refugees
