Job Summary
Conduct home visits, phone calls, and office visits to provide case management support for individuals and families experiencing chronic homelessness. Perform client interviews, assessments, and collateral contacts while consulting with mental health and primary care providers. Develop and modify quarterly service plans to address clinical, supportive, and housing needs, ensuring clients maintain positive tenancies. Transport clients to medical appointments and food pantries as needed, while advocating for resources and coordinating with landlords. Maintain accurate case records in internal and external databases, adhering to all mandatory deadlines and program policies.
Required Qualifications
- BS degree in Human Services or related field
- 3 years relevant work experience
- Must have a valid driver's license
- Reliable daily transportation
- Must be able to lift, move and carry 25 lbs
- Massachusetts Criminal Offender Record Information (CORI)
- Name-based criminal record check (CORI)
- Must be legally authorized to work in the United States
- Employment eligibility will be verified through E-Verify upon hire
- Compliance with Action Inc.'s Driver Safety and Insurance Policy
- pre-employment and annual MVR screenings
- proof of required auto insurance
- Ability to work with clients in crisis and those actively engaging in substance use
- Ability to practice effective de-escalation techniques
