Job Summary
The Housing Case Worker supports clients toward permanent housing by developing and implementing individualized service plans, assessing strengths and needs, and coordinating with community partners. Responsibilities include initiating and participating in case conferences, advocating for access to services, assisting with daily living and safety skills, and maintaining confidential records. The role requires a two-year college diploma (degree preferred), relevant crisis intervention training, a valid Ontario driver’s license, and experience in working with homeless or marginalized populations. The position is full-time, based in Kirkland Lake, Ontario, with a rotating schedule that may include evenings and weekends, and travel as needed.
Required Qualifications
- Minimum of two-year College Diploma in the field of human and/or social services
- Undergraduate University Degree in the field of human and/or social services preferred
- First Aid/CPR and Non-Violent Crisis Intervention or willingness to acquire one
- Crisis intervention and Prevention Training; Applied Suicide Interventions Skills and Training are an asset
- Valid Ontario Class “G” Driver’s License, personal vehicle required and insurance
- Experience: 1-2 years in community and program development; working within cross-organizational teams
- Experience with homelessness, mental health and addictions; knowledge of housing stability and housing-first philosophy
- Knowledge of confidentiality, documentation, and OH&S obligations
- Ability to work independently and as part of a team; strong communication and organizational skills
- Ability to apply harm reduction and trauma-informed care principles
Desired Qualifications
- Experience with homeless/marginalized populations including those with mental health and addictions
- Knowledge of determinants of health and implications to wellbeing
- Ability to work independently and as part of a team
- Strong listening, oral/written communication, and adaptability
- Willingness to complete background checks and screenings
