Job Summary
Conduct eligibility screenings and home visits to assess unique needs of at-risk individuals facing imminent homelessness risk. Coordinate supportive services by connecting clients to housing resources, rent assistance, employment services, and counseling while utilizing the EMPath matrix and Homeless Management Information System. Assist families in establishing housing goals, modify plans as needed, and maintain precise documentation of case management services and program data. Meet with clients bi-monthly to monitor progress and handle crisis situations such as eviction threats. Perform unit inspections to ensure compliance with HUD housing standards.
Required Qualifications
- Bachelor degree in social work, psychology or related field
- Two (2) years of case management experience within the homelessness services, housing programs, or social service
- Valid Texas Driver's License
- NSPIRE Inspector Training (provided by agency- must pass within first 90 days of employment)
Desired Qualifications
- One (1) year of case management experience preferred
- Experience working with clients with a trauma informed mindset
- Knowledge and experience of housing issues and available resources
- Knowledge of EMPath case management model
- Experience and knowledge of the challenges of and resources for those at-risk of homelessness
- Ability to work with diverse communities
- Excellent organizational skills and an ability to self-motivate
- Excellent written/verbal communication and people skills
- Intermediate MS Office skills: including Word, Outlook, and using internet
- Ability to work under pressure, address multiple priorities and meet deadlines
- Ability to identify key issues in problem solving and implement effective and creative solutions
- Knowledge of housing resources, social services, and community resources
