Job Summary
Screen and perform psychosocial assessments for incoming residents, then complete new resident intakes and develop individualized care plans with documented goals. Provide ongoing support regarding program progress, behavioral issues, and life skills including activities of daily living, while leading educational courses, developmental groups, and community meetings. Connect residents to external agency resources and collaborate cross-functionally to facilitate transitions into independent living. Maintain comprehensive documentation for all services and develop discharge plans and exit strategies. Requires five years of clinical social work experience, a Bachelors in Social Work or Ministry, and Texas licensure.
Required Qualifications
- Texas Licensed Master Social Worker (LMSW) preferred; Minimum Licensed Bachelor Social Worker (LBSW)
- Proficient in MS Word, Excel and Outlook
- Ability to work occasional flexible hours as requested
- Five (5) years of clinical Social Work/Case Management experience preferred
- Experience serving the unsheltered community, particularly veterans and families
- Bachelors in Social Work or Ministry
- Direct experience case managing homeless persons
