Housing Case Manager

Job Summary
Conduct intake assessments and develop individual service plans for families experiencing homelessness to secure permanent housing and supportive services. Provide weekly case management, crisis intervention, and advocacy while coordinating referrals for medical, mental health, and employment resources. Perform site visits of prospective units, maintain accurate HMIS records, and negotiate rent within Fair Market Rate guidelines. Build relationships with landlords, community organizations, and participants to ensure housing stability. Submit written reports to management and present cases during supervision meetings.
Required Qualifications
- Bachelor's Degree, from an accredited College or University in Human Services (e.g., psychology, social work, sociology or related field)
- Minimum of two-years experience providing services to persons experiencing homelessness, preferably families with minor
- Demonstrated experience in working with people experiencing behavioral health issues, such as mental health, substance use, sex trafficking, or special
- Experience working with a diverse population, with sensitivity to differences in culture, religion, sexual orientation, race, age, and physical and mental
- Understanding of best practice approaches including: Housing First, Motivational Interviewing, Harm Reduction, and Trauma-Informed
- Knowledge of community
- Strong written and verbal communication
- Ability to maintain personal and professional boundaries
- Knowledge of Trauma-Informed Care and Practices
- Ability to work collaboratively

- Bilingual (English/Spanish)
- Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook)
