Housing Case Manager

Job Summary

Conduct comprehensive client assessments and provide tailored case management, advocacy, and support to individuals and families experiencing or at risk of homelessness. Develop and implement holistic, strengths-based case plans and sustainable housing solutions, including assistance through the Private Rental Assistance Program to help prevent and end housing crises. Coordinate referrals and connect clients with community services, employment pathways, and other supports to enhance wellbeing and self-sufficiency. Maintain accurate client records, case notes, and reporting requirements while contributing to a collaborative team environment. This permanent part-time role (30.4 hours per week) is based in Horsham, VIC, within The Salvation Army's Specialist Homelessness Services team.

Required Qualifications

  • Relevant tertiary qualification in Social Work or a related human services field, or equivalent demonstrated experience in case management and client support
  • Demonstrated knowledge of homelessness including context, causes, issues, philosophies and legislation
  • Ability to apply homelessness models, approaches, theories and practices when dealing with clients
  • Ability to understand and respond to the needs of individuals experiencing homelessness or at risk of homelessness
  • A current VIC Driver's Licence
  • A current Employee Working with Children Check
  • Applicants for all roles require a mandatory Nationally Coordinated Criminal History Check
