Job Summary
Conduct monthly home visits and apartment inspections for residents living with HIV and co-occurring conditions, providing supportive counseling, crisis intervention, and care coordination. Develop individualized service plans, assess financial management needs, and facilitate access to external services including primary care, mental health, and harm reduction programs. Maintain meaningful engagement through motivational interviewing, accompany clients to appointments, and complete all required documentation and reports by prescribed deadlines. Participate in weekly supervision and multidisciplinary case conferences to ensure housing stability and independent living skills.
Required Qualifications
- Bachelor's degree in social work, psychology, counseling, or a related field
- A minimum of three years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, and/or substance use
- Ability to prioritize and multi-task
- Experience with and proven commitment to working with low-income and marginalized communities, as well as individuals engaging in substance use, and having multiple chronic medical and/or behavioral health conditions
- Ability to document in various electronic systems to chronicle client services
- Dependable and flexible
- Able to maintain a high level of client confidentiality
- Excellent internal and external customer service skills and strong interpersonal skills
- Good communication skills, both verbally and in writing, including the ability to communicate with all levels of management
- Must be proficient in Microsoft Outlook, Word, and Excel
- Ability to work with colleagues to solve problems
- Familiarity with Harm Reduction practices
- Present in the program site 100% of the time
- A working knowledge of NYC mass transportation systems (subway and bus)
