Job Summary
Develop individualized service plans and provide intensive case management for young adults with disabilities experiencing homelessness, focusing on housing stability and self-sufficiency. Facilitate life skills training, connect participants to community resources including healthcare and employment, and conduct assessments to monitor progress and support crisis intervention. Maintain accurate documentation in HMIS systems, provide transportation to appointments, and participate in community outreach and partnerships. This role requires a Bachelor's degree in Social Work or Human Services, experience with youth homelessness, and proficiency in Microsoft Office. Full-time position with occasional evening, weekend, and rotational on-call duties in West Palm Beach, FL.
Required Qualifications
- Bachelor's degree in Social Work, Human Services, or related field
- Valid FL driver's license
- Ability to pass drug screen and a Level 2 background check through DCF Clearinghouse
