Job Summary
Provides intake, psychosocial and substance abuse assessments and develops individualized treatment plans for clients in an assigned caseload. Delivers case management services to support adherence to Service Plans, conducts home visits, coordinates medical/psychiatric/substance abuse services, monitors medication compliance, maintains case notes in AWARDS/CARES, conducts individual and group supportive counseling, educates tenants on topics such as money management and employment opportunities, links clients to community services, and participates in weekly clinical meetings to discuss high-risk clients; performs other duties as assigned.
Required Qualifications
- Bachelor’s Degree in Social Work or related field of experience required
