Job Summary
Housing Case Manager supports Plymouth residents in a unionized PSH setting, maintains housing stability via Housing Service Plans, coordinates care with residents and internal teams, assists with scheduling and transportation, advocates with community providers, and ensures a safe, collaborative building environment. Responsibilities include building strong resident relationships, collaborating with internal behavioral health teams, maintaining documentation and databases, providing backup support as needed, and promoting trauma-informed, harm-reduction housing-first principles.
Required Qualifications
- BA/BS degree in a human service field
- Minimum of two (2) years of experience working with homeless and low-income individuals
