Job Summary
Meet with referred individuals and Housing Locators to coordinate moves into housing, while developing and updating Housing Stability Plans every three months. Conduct monthly home visits to support transitions, assist with transportation for health appointments and interviews, and identify barriers to stability through creative solutions. Maintain organized documentation for move-in and recertification, connect clients to social and benefit resources, and ensure compliance with HUD requirements. Develop recovery-oriented Individual Rehabilitation Plans, resolve crisis situations, and participate in case reviews and county committee meetings. Maintain accurate accounting for fund disbursements and collaborate effectively with external agencies. This full-time role requires on-site presence, driving a personal or agency vehicle, and working in the Fort Washington, PA community.
Required Qualifications
- Bachelor's degree in a related field including human services, social work, psychology or special education
- Certified Peer Specialist certification
- Successful completion of Certified Psychiatric Rehabilitation Practitioner certification
- department approved twelve-hour psychiatric rehabilitation orientation course
- One year of relevant experience in housing support or direct supportive services
- Ability to communicate well

- A valid US driver's license for at least two years, not including time with permit
- No serious traffic convictions within the past three years
- Access to a reliable vehicle during work hours
- Valid auto insurance
- Ability to lift or move items associated with sedentary and light work (exert up to 20 lbs.)
