Housing Case Manager II, DBHDD HSP

Job Summary
Conduct Medicaid billable intensive case management for a caseload of 25-40 chronically homeless adults to obtain and maintain permanent supportive housing. Perform outreach, intake, housing searches, lease-ups, and ongoing in-home services while collaborating with community partners and property management. Document all services using the GIRP note format in Welligent, monitor productivity targets, and provide transportation to teach independent living skills. Work within a Housing First team applying trauma-informed care and recovery-oriented models to support mental health management and self-sufficiency.
Required Qualifications
- Bachelor's degree from an accredited college or university
- Associate's degree and the minimum 2 years of human services experience
- A minimum of 2 years related human services experience providing case management services serving chronically homeless adults struggling with a mental illness
- Minimum of 2 years providing social services case management which included collaborative client-centered support that assessed needs, established care plans, and connected individuals to vital community resources guiding clients through complex systems to promote long-term stability, safety, and independence
- Experience writing structured progress notes (GIRP or similar note format) and documenting services delivered timely
- Proficient use of Office 365 products
- Georgia Driver's License with a good driving record
- The ability to pass background checks
Desired Qualifications
- Experience delivering Medicaid billable and/or social service case management services in client's home and/or other field-based community settings
- Knowledge of State and/or federally funded housing voucher programs
- Familiarity with meeting specific productivity standards
