Job Summary
Conduct monthly home visits and comprehensive case management for assigned residents, including outreach, referrals, assessments, and service plan development. Manage the caseload by coordinating apartment inspections, housing plans, and case conferences while maintaining strict chart compliance and regulatory documentation. Provide referrals and follow-ups for primary care, substance use, nutrition, mental health, and financial management, assisting clients with benefits like SNAP, SSI, and rental subsidies. Support client transitions to stable housing by developing individualized goals and accompanying clients to community appointments as needed. Participate in weekly supervision and complete all required reports before prescribed deadlines.
Required Qualifications
- Bachelor's degree preferred in social work, psychology, counseling, or related field, or three years of relevant experience in a social service setting", "Experience with and proven commitment to working with low-income and marginalized communities, as well as individuals with multiple chronic medical and/or behavioral health conditions
