Job Summary
Coordinate and deliver direct case management and stabilization services for clients in the Supportive Housing Program; provide housing advocacy and linkage services to support retention in permanent housing; develop and monitor service plans; provide advocacy, crisis intervention and counseling; coordinate with internal and external reports/databases; develop, coordinate and facilitate groups/workshops; maintain documentation and records; establish service linkages with local providers; arrange transportation to housing, legal, and medical appointments; participate in clinical case conferences and care coordination; conduct home visits to support goals and ensure lease/program compliance; facilitate follow-up/outcome measurement for up to 12 months post-discharge; provide ongoing life-skills training and connect participants to medical/mental health services; ensure access to entitlements and participate in trainings; work as part of a team to deliver services within the Housing Forward program.
Required Qualifications
- Bachelor’s degree in social service or related field
- Master’s degree preferred
- Must have car and valid driver’s license
- Proof of insurance
- Smartphone for agency communication
