Job Summary
Assess housing barriers for families experiencing or at-risk of homelessness to determine service needs and develop comprehensive case management plans. Utilize the Housing First model to provide stabilization services, prevent homelessness, and divert households from emergency shelter through mediation and advocacy with landlords. Conduct weekly home inspections for shelter participants and teach maintenance skills while making necessary shelter and home visits. Maintain accurate case data using HMIS and other automated systems to track goal attainment and outcomes. Advocate for families to obtain essential services including mental health, financial assistance, and medical care. Work an adjusted schedule including evenings and weekends as required, traveling to Fairfax County locations for meetings and events.
Required Qualifications
- Bachelor's degree
- Social work/case management experience with homeless and/or at-risk populations, or experience working in an emergency shelter setting, or other similar case management experience
- Knowledge of and belief in 'Housing First' and 'Rapid Re-Housing' philosophy and strategies
- Ability to work independently in a fast-paced environment and manage a caseload with minimal supervision
- Excellent data entry and data quality skills to establish and maintain case records and to facilitate data collection
- Ability to communicate clearly and concisely, both orally and in writing
- Proficient with Microsoft Office Suite or related software, including Microsoft Excel
- Bilingual Spanish/English
- Ability to work a flexible schedule
- Ability to schedule and manage workload
- Must pass criminal background and Child Protective Services background checks
- Must be able to lift 15 pounds at times
- Must be able to attend occasional meetings at other FACETS office locations within Fairfax County, including 3 annual organization-wide fundraising events each year
