Housing Care Coordinator

Develop individualized housing and self-sufficiency plans including job search, credit repair, and budgeting strategies. Coordinate wrap-around services for health and medical needs while conducting monthly progress monitoring and evaluation. Perform intake assessments for applicants and manage HMIS data entry within 24 hours of service occurrence. Assist clients in accessing counseling, job training, and long-term housing options. Maintain required documentation for accreditation standards and grant requirements. Support staff training and community partnerships to enhance local resources.
