Job Summary
The Housing Based Case Manager CM II provides critical support to individuals experiencing chronic homelessness or precarious housing by developing individualized care plans, conducting assessments, and facilitating housing stabilization services. Candidates should have a Bachelor’s degree in social work or a related field, with a minimum of two years of case management experience. Strong communication, organizational skills, and experience in crisis intervention are essential. The position requires maintaining a caseload of clients, collaboration with community agencies, and documentation of client interactions for performance improvement.
Required Qualifications
- Bachelor’s degree in social work or related field
- Minimum of two years directly related experience in case management
- Knowledge/experience working with individuals in crisis
- Excellent written and verbal communication skills
- Familiarity with electronic data collection and Microsoft applications
