Job Summary

Conduct street outreach with partners in an automobile to engage individuals experiencing homelessness, determine eligibility for Street to Home services, and secure housing solutions. Build relationships with chronic clients using motivational interviewing, while utilizing available resources for non-chronic placements. Respond to 311 calls, participate in Code Blue and Code Red alerts, and monitor encampments to eradicate hotspots. Maintain active registries of chronic and non-chronic clients, perform vulnerability assessments, and enter all required data into outreach databases. Collaborate with the Parks Department, NYPD, DSNY, MTA, and other agencies, while attending clinical supervision and training. Work a 35-hour week with a valid driver's license.

Required Qualifications

  • Two years related work experience
  • Bachelor's degree or equivalent education and experience
  • Knowledgeable about local benefit agencies
  • Familiarity with substance abuse issues
  • Familiarity with mental health issues
  • Familiarity with employment issues
  • Strong oral and written communication skills
  • Proficiency with Microsoft Office Suite
  • Proficiency with electronic record keeping
  • Valid NY, NJ, CT, or PA driver's license
  • Good driving record

  • Bi-lingual (English/Spanish)
