Job Summary
Provide RN care in a 142-bed rehabilitation and skilled nursing facility. Perform comprehensive resident assessments and develop individualized care plans; administer medications and treatments; monitor and report changes in condition. Supervise and support LPNs and CNAs; delegate tasks appropriately. Maintain accurate documentation in compliance with CMS, state, and facility regulations; communicate effectively with physicians, residents, families, and interdisciplinary teams. Participate in care planning, discharge coordination, and quality improvement initiatives; promote infection control, resident safety, and overall quality of care. Sign-On Bonus available for Full-Time status; weekend availability required; shifts include day, evening, and night.
Required Qualifications
- Current, active New York State Registered Nurse (RN) license
- Current CPR/BLS certification (or willingness to obtain upon hire)
- Experience in geriatric, long-term care, or skilled nursing preferred
