Job Summary
In-home health assessments for Medicare Advantage and other plan members conducted by an NP or PA on the Optum HouseCalls team. Perform comprehensive in-home evaluations, document diagnoses for care management, educate members on medications and disease processes, assess social determinants of health, and coordinate with primary care providers. Travel across Wisconsin with provided expenses covered; flexible scheduling within a 8am-8pm window; non-prescribing field-based role focused on gap closure and care coordination; requires active professional licensure and clinical experience.
Required Qualifications
- Active ANCC or AANP certification for NPs (Family, Adult, Geriatric, or Adult-Gerontology Primary Care) OR Current NCCPA certification and state licensure for PAs
- Active, unrestricted licensure in the practicing state
- 1+ years of clinical experience (family, geriatric, or home health preferred)
- Active and unrestricted driver's license
- Access to reliable transportation for home visits
- Ability to navigate varied home environments and perform assessments
- Travel willingness to statewide Wisconsin (~85%)
- No on-call, weekends, or holidays requirements
