Job Summary
HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducts comprehensive in-home health assessments for Medicare Advantage and other plan members across Wisconsin (~85% travel). Role is non-prescribing and field-based, focusing on education, gap closure, and collaboration with primary care providers to improve health outcomes. Primary responsibilities include history review, vital signs, physical exams, screenings, documentation of diagnoses for care management, communication with PCPs, identifying urgent situations, patient education on diseases and medications, and addressing social determinants of health with referrals as needed. Requires active NP PA certification and state licensure, 1+ years of clinical experience, and a valid driver’s license with reliable transportation. Travel and scheduling offer flexibility; no on-call, weekends, or holidays. Salary ranges from $91,700 to $163,700 annually based on full-time employment, with benefits and incentives offered.
Required Qualifications
- Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- Current NCCPA certification and state licensure (for PAs)
- Active, unrestricted licensure in the practicing state
- 1+ years of clinical experience (family, geriatric, or home health preferred)
- Active and unrestricted driver's license and reliable transportation for home visits
- Ability to navigate varied home environments; ability to sit/stand/kneel as needed for assessments
- If practicing in a state allowing independent NP/PA practice, obtain required approval from licensing board within specified timelines
