Job Summary
HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) conducts in-home health assessments across Wisconsin, focusing on Medicare Advantage and other plan members. Responsibilities include comprehensive in-home assessments (history, medications, vitals, physical exams), evidence-based screenings and point-of-care testing, documenting diagnoses for care management, communicating findings to PCPs, recognizing urgent situations and intervening, educating members on diseases and medications, and addressing social determinants of health with referrals as needed. Travel statewide (approximately 85%), living anywhere in Wisconsin, with a flexible 8am-8pm schedule, no on-call, weekends, or holidays; role emphasizes collaboration with PCPs and a team-based care model within Optum HouseCalls.
Required Qualifications
- Active ANCC or AANP certification for NP or Active NCCPA certification for PA
- Active, unrestricted licensure in the practicing state
- 1+ years of clinical experience (family, geriatric, or home health preferred)
- Active and unrestricted driver's license and reliable transportation for home visits
- Ability to navigate varied home environments and perform assessments
- If applicable, eligibility to practice independently per state requirements and timing to apply for licensing approvals
