Job Summary
Coordinate client care, appointments, transportation, and documentation while providing direct support to ensure services are safe, organized, and person-centered. Monitor and document required health information, manage approved client spending activities, and maintain a safe, clean, and well-supplied home or program environment. Train and support employees on client-specific care, routines, and program procedures while communicating health, behavioral, safety, and staffing concerns to the appropriate team members. Participate in client reviews, planning meetings, and treatment team meetings. Maintain confidentiality, professional boundaries, and respectful communication. Work closely with employees, program leadership, healthcare providers, and other members of the individual's support team.
Required Qualifications
- High school diploma or equivalent
- At least two years of experience supporting individuals with intellectual or developmental disabilities or in a related human services setting
- Valid driver's license and acceptable driving record
- Ability to obtain and maintain required medication certification, CPR/First Aid, behavioral training, and other MME-required credentials
- Basic computer, email, and electronic documentation skills
- Strong communication, organization, time-management, and problem-solving skills
- Ability to work independently, manage multiple priorities, and remain calm during emergencies
- Reliable, responsible, compassionate, and committed to person-centered support
- Ability to meet the physical requirements of the position
- Ability to occasionally lift or move up to 50 pounds and participate in two-person client transfers
Desired Qualifications
- At least one year of leadership, supervisory, or staff-training experience
- College coursework in human services or a related field
- CNA, PCA, or comparable direct-care experience
